Free Part A Premium

  • For most people: $0 If applicable
    If you or your spouse worked and paid into Medicare through payroll taxes for 10 or more years, you pay nothing each month for Part A

When you turn 65, you may be eligible for premium-free Part A based on your spouse’s work history if:

  • You are currently married and your spouse is eligible for Social Security benefits (either retirement or disability). You must have been married for at least one year before applying.
  • You are divorced and your former spouse is eligible for Social Security benefits (either retirement or disability). You must have been married for at least 10 years, and you must now be single.
  • You are widowed and married for at least nine months before your spouse died. You must be single.
  • if you’re qualifying based on your spouse’s work record, you spouse must be at least 62 years old
  • Be aware that there are some exceptions to the eligibility rules listed above.

What happens if I have to pay a premium for Part A?

If you (or your spouse) did not pay Medicare taxes while you worked, and you are age 65 or older and a citizen or permanent resident of the United States, you may be able to buy Part A

You need to have 5 years of permanent residency in order to be eligible for Medicare. If you have less than 5 years of permanent residency you can still be eligible for Medicare if you are married to a green card holder or a citizen who is fully insured for one entire calendar year. 

If the work history was less than 7 If you choose NOT to buy Part A, you can still buy Part B.

Part A premium for people with 30+ (but less than 40) quarters of work history will be $278/month (7-9 years)

Fewer than 30 quarters of work history, the premium for Part A will be $506/month (Less than 7.5 years)

In most cases, if you choose to buy Part A, you must also:

  • Have Medicare Part B (Medical Insurance)
  • Pay monthly premiums for both Part A and Part B

People who don’t qualify for premium-free Medicare Part A can opt to keep their ACA marketplace health insurance rather than sign up for Medicare at 65. You’ll continue to receive subsidies to help pay the premiums, but they end if you enroll in Medicare.

Think carefully before making this decision: If you decide to sign up for Medicare later, you may need to wait until the general enrollment period and have to pay a late-enrollment penalty for Part A in addition to a Part B late penalty.

Part A Penalty

If you have to buy Part A, and you don’t buy it when you’re first eligible for Medicare, your monthly premium may go up 10%.

You’ll have to pay the penalty for twice the number of years you didn’t sign up

If you were eligible for Part A for 2 years but didn’t sign up, you’ll have to pay the higher premium for 4 years.
